Suite Signon With Php Is Not Working, Through My Account Suitelet Connection Point

hi
we are trying to use suite sign on concept.
the situation is: customer can logins to MyAccount on website(netsuite website), then he goes to his customer center window, there we have a suitelet connection point it should direct him to my PHP website.

we done as per documentation, but when we clicks on the connection point it brngs up a blank page not going to PHP page.

what we have done is:
we have created a Suitesignon record and passing global variable as customer _id
then we have created a suitelet with suitesignon id and we have deployed and linked in customer center.
