Subscribe option when using cases to generate leads

We are using a support form which generates a lead.

The problem is that there does not seem to be a way to give that person creating the support case a way to subscribe, they all come in as unsubscribe.

How can we give these people the option to opt in?

I understand we could ask them to login and change their subscribed status, but it's way too hard, easier would be a box "opt in/out" and the first point of contact (the support form).
