Lead reports

I am having a hard time getting accurate reporting on leads. It seems as though if an transactions starts as anything other than a "new lead" it doesn't show up in the lead reports. I have also found that some deals entered as a New Lead also don't show up.

Has anyone found a way to get good accurate reporting on lead tracking?
