What happened to Name WITH address?

In the past whenever we add a new customer, once we type the customer name in, it is always the first line in the customer Address.  For a while there was an option to include the Name or not.

Now with the new release, the name no longer appears in the address box, AND I can't find the option to turn it on.

When someone creates a customer record from our website, the Name appears just like it used to.. just not when we do it from within Netsuite.

Can someone help?
