Tracking Case Assignment

We use cases to track many company processes.  A case could be escalated to three or four departments and be assigned to just as many employees.  We are having trouble tracking the trail, in anything but system notes.  Email notifications of reassignment do not include who has assigned the case.  You can source the author, but that does not show who did the escalation / reassignment.  How are other people dealing with this issue.
