Selecting Multiple Items to one Case

I need to figure out a way to list multiple products (items) in a case.  I would like the interface to look like the Entry for Sales Orders.

I need to capture for a single case:

1.  Item & Quantity, if more then one
2.  Serial #'s for each item
3.  Problem for each item.

I would appreciate any insight.
