NetSuite losing case notes

Has anyone else had NetSuite intermittently lose case notes? We had a rep type up two hours worth of notes on a call and when he saved the case, his notes were gone. No record of them. Nothing in the history or system notes. The rest of the case is saved.

This is not the first time this has happened to one of our reps, but it occurs randomly, at different times of day and with varying lengths of notes.
