Lead Conversion - Missing Title Field

Hi
I have successfully managed to setup lead conversion (Individual -> Prospect & Opportunity). But after making a change I do not remember, I am now missing the Title field that gets populated in the Opportunity.

- where can I find the Lead Convert configuration? How did "Title" all of a sudden disappear?
- Can I add additional standard fields I would want to populate (custom fields I know how to do) on the Opportunity?
