Problem with Simple Form

Hello we have a simple form that captures company and email, registers a lead and provide a discount code. 

The form was working wonderfully and now after updating some graphics on the .html template, we are presented with the correct form but when hitting the submit button are presented with an error

"An unexpected error has occurred. Please go back and try again."

The HTML template has the form area with <NLFORM>, <NLCOMPANYNAME>, and <NLEMAIL>  unchanged from before.  Any ideas where to look for the issue?
