Source Sales Rep

We use sales rules to split up web form leads. The lead gets assigned to a sales rep (in our case based on zip code).

I would now also like to create a separate form that would allow the person submitting the form to choose the Sales Rep, rather than having it get assigned via sales rules. The first problem I'm having is getting the Sales Rep to source on the web form. I tried creating a custom entity field, but it doesn't seem to source correctly.

Any tips on getting a sales rep drop-down list to display on a web form?
