lead and custome form

ok got a form that will create a new lead etc and send out a confirmation email and have forms that create a custom record and this will then appear on a subtab of the lead record.

what i am wondering is is there a way in which these two functions could be rolled into form? i.e. create a lead with standard info and populate a subtab with custom information.

I've tried to create an online customer form and include the tags for the custom record but thats not working
