Form field with default value

We are using the online web forms on our website (not in NetSuite) for PDF document requests and they work fine.  We'd now would like to know what papers are being requested so we thought we'd add the document title to the form and have it default the value into a custom field in NetSuite.  Not having much luck.  The HTML codes we're using isn't doing the trick.
