Cookies for online web forms

Has anyone used cookies for the online web forms?  We use the forms to capture information from visitors that want to download documents from our website (not in NetSuite).  Some of these visitors download up to 20 documents and have to fill this form out each time.  I know I'd get tired after the second time.

Any suggestions?
