"Contact Role" in Online Customer Form?

We want to survey our customers and ask them the question:  "What is your role within the company?"  Using the Contact Role list as a selection.   

The Role field is not an available field to choose from when building an online customer form.  How can I give the visitor the option to choose one of the Roles from a list when filling out the form? 

Also, we generally have multiple contacts for any one company.  Any ideas on how we can capture more than one contact at a time on a form?
