Custom HTML Form Problems

I'm trying to set up a form for new customers (that are not in our database) to sign up for a newsletter for different products of their choosing.  I want them to type in their email, first name, and last name, then check the product boxes that apply. Then all their information will be added to our customer database.

The problem I'm having is that I can't get the pre-made customer fields (first name, last name, email) to show in the custom form.  I put the tags in the template correctly but I can't add the fields to the custom record.
