Online Case form-Help needed

Okay.....so here's the deal;
I have a Customer who has multiple locations. I have created a form for them to submit cases online. However, matching the customer by their email address will NOT work in this Form. The reason being that I have one contact over seeing 2 or more Sub Customers. Is there a way to match by Company Name?

I was thinking of a "Pick List" that they can access that has all the sub-customers and have them select the appropriate one but I am unsure how to go about this. There are approx. 100 sub-customers.

Would this be a doable?
