Capture Billing and Shipping Addresses with Online Customer Form

Is it possible to capture two addresses with an online customer form (one marked shipping address, and the other marked billing address)?

When I create an online customer form in the UI it only gives me an option for one address (although separated into fields of Address 1, Address 2, City, State, Zip, etc)
