Creating an online form that feeds data to the Customer Account

Hello,
I would like to create an online form/page that I can send to a customer to fill out, and when the form is complete the data is populated to the Customer Account page. 

The specific use case is a Customer Profile questionnaire, and I want to know if the particular organization offers a certain service.  So if I send this form to my customer "Joe the Plumber," and Joe checks the boxes for New Construction, Repair, and Rotorooting, then on his customer record these boxes (custom check boxes) will be updated and checked. 

Anyone out there ever do this?  Any service providers have an opinion on how this can be done?
