Custom Record Online Form - HTML Template. Where is the Submit Button

I have made a custom online form using the Custom HTML Template and it looks great. However it does not work because there is no submit button. I assume that there is a tag for this but I can't find it. How do I add a submit button to the online form?

My on-line form is capturing first and last names, email, telephone, password, password confirmation and access(hidden).

When it is submitted I want it to create a new lead.
