Using country-specific address form fields in a online customer form

Our company utilizes the Country-Specific Address Forms bundle for international addresses. I'm now creating an Online Customer Form to collect Leads. Currently the fields associated to the Country-Specific Address Forms do not appear in the dropdown list of fields that can be included in an online form. Is there a way to associate or relate these fields to a contact record so that they can be used in a Customer Online Form?
