Auto email on forms not working in V11

Since V11 one of the forms on our web site is not working.  We ask people to sign up a free training webinar by filling out a form on our web site.  Before v11 the customer would get an email with confirmation and we would get an email.  Now we get the email but the customer only gets the error message, "An unexpected error has occurred. Please go back and try again" with the NetSuite  logo above it.  So they keep trying until they give up or we call them.   We were using the forms found under Setup -> Other
