Email 'from' field always Company default email

It seems that every email generated from NetSuite regardless of where and regardless of what we have for email addresses set up, everytime an email is sent it only contains the default company email address.

This includes emails sent from sales, order acknolwedgements, logins, or from online forms that generate an email reply.

The name may reflect the logged in user or owner but the email address is always the company default address.

What am I doing wrong and how do I correct this?
