Email Subscription Preferences

We are currently in the process of sending out an email to customers to ask them whether or not they would like to opt in or out to marketing emails.
Having designed a nice HTML marketing template that has a nice clear button with in it to click on, which should take them to the 'Email Subscription Preference' page. Now I can't seem to get the right URL that enables them to do this, as although it goes to the correct page their email address is not populated and the submit button is greyed out.
It's works fine in the mandatory footer note on the marketing email that mentions about unsubscribing. So my question is where do I find this URL which links it to the customers email record, so they can submit it?
