Importing items - errors and prices

I am having trouble importing items since version 11.  I keep receiving the following message. 

Errors in item record import:
Record 1: The storeDisplayThumbnailRef element contains a reference that is not unique. Please correct the file and import again. <storeDisplayThumbnailRef> <name>8241.jpg</name></storeDisplayThumbnailRef>


I also find that it is no longer uploading prices.
