Custom Record ID in CSV update?

I'm looking to update a custom record via CSV (using update or add).  Both the name and id fields are enabled on the record.

The problem is that "name" used in our custom record is not always unique.  I receive an error for any records where name is not unique.  Unfortunately, the CSV template does not specify a column for id.  How can I uniquely identify these records using CSV?
