CSV Import Assistant Filed Mapping. Controls Not Working

Teh controls on the page refuse to open up.  Both the CSV fields on the left and the Netsuite fields on the right.  The centre pane is also blank apart from the get Help line.  My laptop is fine but my MD is having the problems.

Help appreciated

OK, it's teh format of the CSV.  Are there restrictions on the column headers?
