CSV Note Import to Custom Record

I am trying to move some historical data from an old home grown CRM into NetSuite. We created custom records to hold our product information but now I am trying to import User Notes to the Custom Record.

I cant figure out how to import them. I know i will have to do it as a user note import but not sure how to link the notes to the specific custom record that note needs attached to.
Example: The custom record is Products - under that custom record we have custom forms for each product (Bidmate, Website etc.)

I need to attach website notes from the old system to the new website product page as a note. Thank you in advance!!!
