Permission Level Change History

When making changes to permissions in custom roles there is a history log in the history tab that reflects all changes made and who made them.  This history does not show up in the system notes and I would like to generate an audit report against these changes.  I know some of the fields are available when creating a saved search for roles, but I can't seem to the user making the change and what the prior permission level was before the change.  Does anyone know what fields those would be?
