Create a Line Item

How do I create a line item?  I see how to get and set existing line items, but I don't see how I can create a new line item.  Can someone post a few lines of code showing how to do this.

In my case I am trying create a purchase order, but I'm sure the same concept would apply to sales order, invoice, etc.
