Setting Sales Tax Rate on new Quote

When generating a quote in a server-side script, I am required to set the sales tax item and sales tax rate.  This raises three problems:

1.  The sales tax item (and rate) should be set automatically depending upon the shipTo on the quote.  So this requirement appears to be a bug.

2.  I can't find any documentation of how to retrieve the rate from either the customer record or the salestaxitem. 

3.  Even if I set the item, I still need to set the rate, even if sourcing is set to true.

Suggestions anyone?
