Error on ServerSide Script on Web Generated Sales Orders

I have a Server-Dide script that runs on all Sales Orders both web and manually generated. This script creates a custom record in an aftersubmit function. It works 95% of the time for web orders, but the other 5% it doesn't work. It is not a problem on manually enter Sales Orders. I have looked through the Sales Order with and without the error and I can't find a reason that the script is throwing the error.

Here is the error I get (the number for invalid reference key is different for every error message):
