enable or disable checkboxes : Script

Can someone help me on this issue ?.



I have 2 checkboxes on my custom form called 'yes' &  'no'. if user clicks on "yes" the second checkbox "no" needs to disabled. if user unchecks the "yes" the check box "no" needs to be enabled.



as well as, if the user checks the "No", the "yes" needs to disabled. if user unchecks the "no". yes needs to be enabled.





can some one send me custom code for this functionality and let me know how to upload  it in my custom form.. so that i can edit it with my ID's and get it to work.
