Execute server event script from Customer Survey Form

Hi,

I have a simple customer survey form with a couple of fields in it. I want to execute some code (send email) when the customer selects the submit button. However the submit does not seem to fire of the code.

Can someone tell me how i deploy my userevent script to run from customer survey forms?

Incidently my server side script code runs fine if run in the normal way from other case forms.
