Building a Custom Record to show in the Customer Form

I want to create a custom record for certain information regarding an existing customer.  By creating a custom record I could build an online form for the existing customer to fill out and capture this data.

Is there a way to create a custom record and have that show up as a sub tab in the customer form?  Also, withthin that sub tab, can I have other sub tabs within that rather than clicking on a link to take me to the custom record?
