Multiple Contacts on one Record Creates Error?

Hi I have created a customer record, in which I have fields for multiple contacts that are filtered by the customer (parent) previously selected on the same record. Now that I have done this is I get an error reading “You cannot submit this form due to an Unexpected error.”   The contact records are also set up to be parents of this record along with the Customer itself.

The purpose of this record is to be a registration form.  Any ideas what the DB does not like?
