How to Display Custom Record on Lead/Prospect/Customer form

Hi,

I have created a custom record to capture details like creation date, expiry date for the product selected by the customers. custom record list will look like this:

Customer Name     User login     Date Created   Expiry date  Last renewal date
XXXX                  123456         10/5/2007       10/04/2008 
XXXX                  789012         10/5/2007       10/04/2008
YYYY                  565456         10/1/2007       09/30/2008
YYYY                  878675         10/1/2007       09/30/2008

What I want to do:

When i go to Lead/Prospect/Customer record for XXXX
In a new tab that i created in Lead/Prospect/Customer Form
the filtered list is shown from the Customer Record like:
Customer Name     User login     Date Created   Expiry date  Last renewal date
XXXX                  123456         10/5/2007       10/04/2008 
XXXX                  789012         10/5/2007       10/04/2008

How can I achieve this?
