Contact Extension

We added a field for the contacts extension.  When editing a contact it is easily updated.  However, when adding a new contact, the extension field is not in the form and there is not an option to customize this form to add the contact extension field to the form.

I am sure there is something simple I am missing.  Any help is appreciated.
