Where is my custom record?

I have created a custom record with no permissions set.  I have also created a sub-tab for the custom record to appear on the lead/prospect/customer form.

The new sub tab appears in the list of sub tabs when customizing the form.  However, the list does not appear on any of the tabs under lists.  Also if I go to 'move items between subtabs', the custom record doesn't show in that list either.

What am I missing?
