creating new customer record from my account tab

Hi,

I created a custom record, one of its field is customer and it is selected as a parent field, and I add a link for this custom record in customer center (The customer can access her/his custom records from "my account" tab).

Now, when the customer go to my account tab, new 2 links appeared, one for the list of custom records and another to create new custom record.

If the customer click to create new custom  record, the custom record created but it leaves the customer field empty, so the custom record didn't appear in customer record

Why this happened, and how can I fix it?
