Custom Images on a PDF for a custom form

Afternoon all

I am having a bit of a dilemma at the moment and am hoping that some of you guys (and gals) might be able to point be in the right direction.

I have a custom record that I have created to use as a quick estimate which makes some calculations based on a small number of variables to get our customers a rough price for our goods without having to go through and work out all of the line items and add them to a NetSuite quote.

One of the variables is a list with 3 options depending on the way the floor is constructed (screed, timber frame etc) - each of these have a diagram to represent them which we would like to show on the output for this custom record. When I place the image field on a PDF template and do a merge, I get just text with the name of the .jpg and no image at all.
