Custom Record for Tracking Head Count

Hello Everyone,

Our Sales rep and SC told us we could build a custom report to track head count over the past few months/yrs.  when we learned that this is not out of the box functionality we were told to create a custom record.  I am new here and don't really know where to start.

Do I build a child record off of the existing employee record?  Do I make this an stand alone record?
What fields should it have?
Has anyone else done something like this?

The report I'd like is :
