Add a field to upload file from custom online form

Hello,

I have added a custom field of type document in a custom record. Also a online form is attached to that custom record. I could able to add all the fields to online form except the field of document type.

Is there any restriction regarding showing a file upload field in online form ?

How can i add a field to attach files from the online custom form to save under file cabinet? Any help would be great.
