Can't set Custom Customer Form to preferred

We created a custom Customer Entry form and set it to preferred, however it only defaults correctly to this form when creating a New Customer Record. When creating a New Lead or New Prospect, the default is still the Standard Customer Form.

Am I doing something wrong? Is there a different form for Leads and Prospects that need to be customized as well?
