One form for viewing, one form for creating

I have a custom record type for employees to enter company requests.  I would like them to enter information (create a new record) on one form, and then be able to view the status of their request on a different form.  Is there a way to limit creating new records only to one form, and then when we view the "status" of this record only allows another form?

The main purpose is to keep it simple to use and prevent mistakes.  Any ideas would be appreciated.
