Changing Ship To in Custom PDF Layout

Before version 2007.0, I customized my Ship To element in a Custom Disclaimer layout.  I added a phone number, contact name and e-mail address.  My memory is fuzzy as to exactly how I accomplished that but I swear there was a second screen in the PDF customization where I could "hard code" these as part of the Ship To information.

Now that I'm on Version 2007.0, I have a need to change the contact name and e-mail address in this Ship To field.  I can't for the life of me figure out where I can do that.  In the new Custom PDF Layout screen, I cannot see any of the custom fields I added to the Ship To.
