Case type in online case form to determine value of custom form

Hi all,

I have an online form and have a field for case type that is to be seleted when the user fills out the form. I have created a custom form for each case type.

When the user submits this form is it possible by the use of server side scripting to set the custom form in NetSuite based on the case type selected in the online case form?
