Case form not "sticking"

We currently have four separate custom case forms that our support reps use to manage different types of issues.  Every time a case is opened with a custom form different then the default it will always revert back to the default when the case is opened again.  How do I get the custom case forms to stick to specific cases and not always revert back to the default case form?
