Online Case Forms applying wrong company name to cases

Background:
On our web page, we want to link to a form that potential customers can fill out to request a quote for our services.
A single company needs to be able to request multiple quotes and therefore will need to be able to submit the form multiple times. For this reason, we are using an Online Case Form rather than an Online Customer Form.
