Secure Online Client Form - possibilities?

Hi

As mentioned in a similar post, we've put up a form that will be used to receive lead data from another CRM system.

Apart from securing the communications, can one lock down access to the form by:
- Username (requiring logon to use it)?
- Client IP?

Any thoughts would be appreciated!
