Unassigned Form being used by Custom Role

I have an Issue logged with NS but need an answer ASAP. I am wondering if this has ever happened to anyone else. And if so, what did you do about it. Does anyone have any suggestions on how to debug it?

I have a Custom Sales Role assigned to all Sales Reps. There is a Mgmt Estimate form that they do not have permissions to use -- Enabled No and Restricted No on the Forms tab in the Custom role and Preferred is Not checked on the Roles tab of the Form, another Estimate form is preferred for this role.
